Compass is a province-wide service that supports evidence based care to all BC children and youth (0-25) living with mental health and substance use concerns. This is done by supporting community care providers with the information, advice and resources they need to deliver appropriate and timely care to children and youth close to home.

 

This is an Indigenous Care Coordinator role for the Compass Program, BC's Children's Hospital province-wide mental health and substance abuse provider consultation service. The team is looking for an adaptable individual to engage with and provide support to a variety of health care providers across the Province bringing an Indigenous lens and approach to care for children and youth with mental health and substance use concerns. Ideally, a successful candidate would have flexibility, adaptability and a "lean-in" attitude. In this role, attention to detail, self-direction, problem solving, initiative, optimism and relationship building skills are essential.


What you’ll do

  • Establish effective working relationships with community based agencies and health care providers who provide services for Indigenous communities to facilitate accessible and effective culturally safe, trauma informed client and family care.
  • Develop and maintain relationships with community care providers and conduct outreach site visits, education and training to these providers regarding the access to culturally safe, trauma informed mental health and substance use care programs, services, and resources available to children, youth and their families in communities around the Province.
  • Work collaboratively with the Outreach and Access Program Team, community care providers, patients and family members around the coordination of services.
  • As needed act as the first point of contact for phone calls/consultations to the community care providers and connect them to the appropriate resources for their patients and families.
  • Provide administrative support, coordinate day-to-day activities and triage functions for the Outreach and Access Program.
  • If and as needed, advocate, assess patient and family need for therapeutic and practical supports through direct telephone contact and assists families in accessing and securing resources in their community.
  • Support addressing emotional and practical problems of moderate complexity; support and promote culturally safe, trauma informed educational and supportive group programs and the development of groups to meet the needs of clients and families.

 

What you bring

 

  • Bachelor’s degree in a relevant Social Sciences discipline.
  • Two (2) years related experience working in a health care environment that includes experience counselling individuals and families and group work or an equivalent combination of education, training and experience.
  • Comprehensive understanding and experience delivering culturally safe, trauma informed care
  • Eligible for registration with designated professional regulatory body.
  • Valid BC Driver’s License. Local area travel may require the use of a personal vehicle.

  • Federal, provincial, territorial, regional and Indigenous organizations mandated to provide mental health and substance use care services.
  • Health issues impacting Indigenous communities.
  • Community-based Indigenous organizations across BC.
  • Family support and child and youth mental health.
  • Community agencies and resources.
