The Licensed Practical Nurse, Sunny Hill, in collaboration with a Registered Nurse (RN), provides care to patients with known health problems and predictable outcomes.


What you’ll do

  • Assess and identify patients’ and families’ actual potential strengths and limitations using physiological, psychological, social-cultural and spiritual needs.
  • Contribute to the development and modification of individualized nursing care plans in     collaboration with the RN and input from the family.
  • Deliver and collaborate in planning and coordinating the nursing care of patients by documenting and updating information about the patient’s condition, reporting problems and/or potential problems to an RN and /or physician and changing the patient’s condition as required, and completing admission, teaching, discharge and transfer sheets.
  • Review and interpret the established nursing plan of care, provide nursing care and assist patients with hygiene, grooming, toileting needs, ambulation and mobility, and feeding, and performs procedures.
  • Follow diagnostic treatment plans including collecting specimens and preparing and caring for patients undergoing diagnostic tests, examinations, or procedures.
  • Participate in and support patient and family teaching to patient care and discharge planning.
  • Participate in the orientation of new LPN staff and LPN students, and assist in the orientation of other members of the health care team.

 

What you bring

 

  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
  • Current full practicing licensure with the British Columbia College of Nurses & Midwives (BCCNM).

 

You also have the ability to:

  • Communicate effectively both verbally and in writing.
  • Deal with others effectively.
  • Physically carry out the duties of the position.
  • Organize work.
  • Operate related equipment.
