Overview

Working within Correctional Health Services, you will provide trauma-informed care to clients presenting with a wide variety and complexity of acute and chronic illnesses/diseases, including communicable diseases, substance dependency, and mental health and general health issues. You will be a member of an interdisciplinary, collaborative health care team, liaising with other health care professionals, clients, family members, and community agencies to provide clients with safe and effective care. Be part of a team where you can live and work in your community, positively influence an incarcerated individual’s journey down a new path, and see the difference you can make as you serve the diverse populations of the BC Corrections system.

About Correctional Health Services

Correctional Health Services (CHS), part of BC Mental Health & Substance Use Services (BCMHSUS), a program of the Provincial Health Services Authority (PHSA), is an integral part of British Columbia’s health care system. CHS provides health services at the 10 provincial correctional centres. PHSA is mandated to improve the quality and continuity of care for clients across the province during their time in correctional facilities and as they reintegrate into the community. This will help to improve access to primary health care, as well as mental health and substance use support for inmates; key needs that have been identified not just in BC, but internationally.

Indigenous Patient Navigator Coordinator, Community Transition Team

What you’ll do

  • Manage an interdisciplinary team of Indigenous Patient Navigators setting goals and objectives and managing performance outcomes; uses effective management techniques (e.g., coaching, mentoring, skill and leadership development, performance management, and cultural competency) to support the achievement of required outcomes and ensure adherence to accreditation, regulatory, and safety standards; conducts investigations involving staff misconduct and implements disciplinary action up to and including dismissal in conjunction with the Director. Provide advice to leadership on labour disruption contingency planning strategies.
  • Provide input to the Director in developing an effective workforce plan and makes adjustments in collaboration with key partners that ensures the availability of adequate staff within the context of operational demand, and environmental and resource constraints. Monitor requests for overtime, vacation, leaves of absence, and sick time for all designated staff.
  • Collaborate in program planning processes including the development and implementation of innovative and creative approaches for delivery of high quality, value added and efficient inter professional services to support long term strategies.
  • Collaborate in the development of annual capital and operating budgets to complement strategic directions; administers the department operating budget to adhere to allocated budget by monitoring expenditure, taking corrective action, and producing rationale for deficits.
  • Coordinate the quality improvement and risk management activities and facilitates the implementation of approved modifications and evaluation of process effectiveness to ensure optimal use of organizational resources and safe delivery of patient care.
  • Conduct statistical, workload and financial analyses, monitor patient flow and make recommendations in support of the department’s business plan and financial forecasting model.
  • Participate in other program and project activities as assigned by the Director.

What you bring

Qualifications

  • A level of education, training, and experience equivalent to a Bachelor’s degree in Nursing, Health Sciences, Justice or relevant clinical discipline plus a minimum of five (5) years of recent, related experience working with Indigenous communities in a hospital or public health care environment in an administrative/clinical capacity with direct supervisory experience. Occupational First Aid Level 2 required.

Skills & Knowledge

  • Established knowledge and network of Indigenous communities, services, agencies throughout BC.
  • Ability to direct and respond to inquiries of Indigenous Health department alignment to Paige’s Story (2015) and the Truth and Reconciliation Call to Action report (2015).
  • Ability to plan, develop and implement protocols in relation to treatment programs and patient/family priorities.
  • Comprehensive knowledge of the principles of Indigenous-centred care. Knowledge of the needs of women and families with health conditions and the ability to advocate for patients as they navigate the health care system.
  • Strong operational management skills and demonstrated understanding or experience with operational planning and program management. 
  • Critical thinking skills and proven ability to effectively manage human and fiscal resources.  Proven leadership ability, with communication, interpersonal skills, and an Indigenous cultural lens conducive to interacting in an inter-disciplinary environment and liaising with staff, patients and families. 
  • Demonstrated facilitation, conflict resolution and problem-solving skills. 
  • Ability to operate related equipment including word processing, spreadsheet, and database programs.
  • Physical ability to perform the duties of the job.
  • Commitment to develop knowledge and understanding of legislative obligations and provincial commitments found in the foundational documents – including Truth & Reconciliation Commission’s Calls to Action (2015), In Plain Sight (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), United Nations Declaration on the Rights of Indigenous Peoples (UNDRIP), Reclaiming Power and Place Missing and Murdered Indigenous Women & Girls Calls for Justice (2019), the Declaration Act Action Plan and Remembering Keegan: A First Nations Case Study – and how they intersect across the health care system.
  • Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).

Job Type: Regular, Full-Time
Salary Range:
$88,990 – $127,923/Year. The starting salary for this position would be determined with consideration of the successful candidate’s relevant education and experience, and would be in alignment with the provincial compensation reference plan. Salary will be prorated accordingly for part time roles.
