What you’ll do
- Assess patient and family needs for practical supports and assists families in accessing and securing resources internal and external to the hospital.
- Assess families’ needs and provides support services such as transportation, parking, and meal vouchers. Assist families in accessing and completing appropriate forms and applications.
- Provide support with education to patients and families related to their medical and/or psychosocial needs.
- Provide community resource information and ongoing support services to patients, families and health care professionals, and facilitate referrals to community resources.
- Assist the program staff with managing and communicating information on community resources, and creating systems to update and communicate this information to patients and families.
- Participate in quality improvement activities including reporting unsafe situations and reports as per provincial and organization’s policies and procedures.
- Participate in the planning and implementation of special events and in the organization of activities for patients and families as needed.
- Support research activities within the program as necessary.
- Participate in team meetings and in other meetings relevant to the program.
- Perform other related duties as assigned.
What you bring
Qualifications
- A level of education, training, and experience equivalent to a Bachelor’s degree in an allied health discipline
- Minimum of one (1) year experience in a health care setting working with a wide range of service providers.
Skills & Knowledge
- Knowledge and experience in family support and pediatrics.
- Extensive knowledge of community agencies and resources.
- Ability to liaise effectively with staff at all levels of the organization and with partners in other health centers and community agencies.
- Broad knowledge of Trauma Informed Care including Collaborative Problem Solving.
- Excellent interpersonal, oral and written communication skills including training in reflective listening and validation.
- Ability to work independently with limited supervision.
- Self-directed and highly motivated with excellent organizational skills.
- Demonstrated sensitivity, confidentiality, and commitment to family-centered care.
- Ability to collaborate effectively within a multidisciplinary care team.
- Knowledge of Indigenous cultural safety, to provide health care services that are equitable, respectful and safe.
- Ability to operate related equipment including word processing, spreadsheet and database programs at an intermediate level.
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
- As a strong asset for consideration, we are looking for our successful candidate to have: Knowledge of social, economic, political and historical realities of settler colonialism on Indigenous Peoples and familiarity with addressing Indigenous-specific anti-racism, anti-racism and Indigenous Cultural Safety and foundational documents and legislative commitments (The Declaration Act, the Declaration Action Plan, TRC, IPS, Remembering Keegan, etc.).
